Abstract:
BACKGROUND: Caregiver violence is an important issue in mental health and it is not well studied. It has universal ethical, legal, socio-cultural, economic and health consequences in Ethiopia.
OBJECTIVE: To assess the proportion of caregiver violence and associated factors among patients with severe mental disorder at Tibebe Ghion Specialized Hospital (TGSH) and Felege Hiwot Comprehensive Specialized Hospital (FHCSH) northwest Ethiopia.
METHODS: Cross-sectional study was conducted among 394 peoples with severe mental disorder who were selected through systematic random sampling. The data were selected through face-to-face interview. Pretested was done and semi-structured questionnaires (National crime victimization survey and World Health Organization multiculturally validated domestic violence tool) were used to collect data. Data was collected by Epicollect-5 and was exported to SPSS Version 25 for analysis. Multiple logistic regression analysis with adjusted odds ratio and 95% CI and p-value (<0.05) was applied to identify significant variables.
RESULT: The proportion of caregiver violence was 36.3% (95% CI=31.5%, 41.3%). From the participants who exposed to caregiver violence (n=143), 50.3% (72) reported caregiver violence in their life time. Average monthly income <2000 ETB (AOR=3.31; 95% CI 1.33, 8.24), having comorbid illness (AOR=3.45; 95% CI 1.51, 7.98), number of episodes ≥2 (AOR=6.29; 95% CI 3.59, 11.02), poor medication adherence (AOR= 5.06; 95% CI 2.29, 11.15) and duration of illness more than a year (AOR=4.17; 95% CI 2.19, 7.19) were the significant factors associated with caregiver violence.
CONCLUSION: Caregiver violence was more than 1/3rd (36.3%) among people with severe mental disorder. This needs public health attention and ethical considerations. Income, comorbid illness, number of episodes and medication adherence were factors significantly associated with caregiver violence.
