Abstract:
Background: The majorities of the world’s perinatal deaths occur during intrapartum period and are avoidable with better care. In Ethiopia, key barrier to reducing maternal mortality is lack of access to quality care during and after childbirth. But studies to assess quality of intrapartum care in the study area are limited.
Objective: To assess the quality of intrapartum care among skilled birth attendants and mothers in public health facilities of North Achefer District, Amhara region, Northwest Ethiopia.
Methods: Institutional based cross-sectional study triangulated with qualitative method was conducted. A total of 216 women and 44 skilled birth attendants were included. The data were collected from November7toDecember6/2019.Both Interview and self-administered structured questionnaires for women and skilled birth attendants respectively as well as semi-structured observation guides were used to collect quantitative data. Focus group discussion guide for skilled birth attendants and interview guides for women were applied to collect qualitative data. Bivariable and multivariable logistic regression analysis were used to identify factors associated with the quality of intrapartum care. P-value less than 0.05 was used as a cutoff point to identify significant variables in the finally model and OR with 95%CI was used to see the strength of association.
Result: Fifty nine (27.3%) mothers were received good quality intrapartum care. Long distance from mothers’ home to the nearby health facility AOR= 0.19(95% CI=0.06, 0.66), care given at health center AOR= 0.07(95% CI=0.02, 0.20) and use of partograph to follow the progress of labour AOR=4.9(95% CI=1.82, 13.14) were associated with the quality of intrapartum care.
Conclusion: Quality of intrapartum care is poor in the study area compared to the national standard and it is associated with long distance from mothers’ residents to the nearby health facility, appropriate use of partograph to follow the progress of labour and type of health facility care given. Therefore, giving on job training on partograph usage to health care providers, avail ambulances to remote areas and make networked road in the remote rural areas are also important.
