Background: One of the potential risks of induction is the failed induction of labor. In Ethiopia more than one-thirds of the mother faces failed induction of labor. The consequence of a failed induction that usually results in a Cesarean-section. Compared to vaginal birth, it is more potential health risks to the woman and the baby, as well as, significantly longer recovery period for the women. Objectives: To assess the proportion of failed induction of labor and associated factors among induced women in Bahir-Dar city health facilities, North West Ethiopia, 2020. Methods: Facility-based cross-sectional study design was conducted among 390 women from March 10 to May 08/2020 in Bahir Dar health facilities. A systematic random sampling technique was used to select the study participants. A face to face interview technique and card review were used to collect data. Six trained data collectors and one supervisor participated in data collection. The collected data were checked, cleaned, and entered into Epi data and then transferred to SPSS for analysis. Variables that have p-value < 0.25 by simple binary logistic regression were included in multivariable binary logistic regression. Variables that have p-value < 0.05 were considered statistical significance association with failed induction of labor. Result: About 35% of induced women had experienced failed induction of labor. Women who lived in the rural area (AOR=2.40, 95% CI: 1.18-4.9), incomplete/absence of follow-up chart (AOR=3.32, 95% CI:1.7-6.47), unfavorable bishop score (AOR=9.71, 95% CI: 4.92-19.18), administration of analgesics (AOR=2.55, 95% CI: 1.32-4.91), and Hypertension disorder as an indication of induction had higher odds of failed induction of labor. Conclusion: The proportion of failed induction of labor is high in the study area. Bishop score status before induction of labor, use of induction follow-up chart, indication for induction residence and analgesic administration during induction were the determinant factors for failed induction of labor. Therefore, performing a pre-induction evaluation of the cervix, and reducing induction if the cervix is unfavorable, strictly following the induction with follow-up chart, and avoiding anti-pain might reduce the failed induction of labor.
