Background Health system governance is a process of directing health system resources utilization and a key to achieving sustainable development goal 3. Ethiopian Ministry of health introduced decentralization principles in Health centers with governance structures. However, there were no studies to show level of governance system functionality among health centers, rumors related to service delivery are common.
