Backgroud: Consequences of pregnancy and childbirth are still the leading causes of maternal morbidity and mortality in developing countries. Antenatal care is one of the most effective interventions in reducing maternal mortality in the developing world. However, there is limited evidence regarding the prevalence of antenatal care utilization and its asociated factors among reproductive age women in the study area. Objective: To asses prevalence Antenatal care services utilization and its associated factors among women of reproductive age groups in Debre Markos town, East Gojam Zone, Amhara, North West Ethiopia. Methods: A community based cross-sectional survey study was conducted from 25th May to 25th June, 2021. A representative women (455) were selected for interview, focusing on reproductive age group category of 15 - 49 years. Data were collected through structured and interviewer administered questionnaires. Five trained data collectors and one supervisor were involved during data collection process. Data collected from field survey workers were entered and analyzed using SPSS Version 25 statistical software package. Frequency distribution, proportion and other statistical analysis was used to analyze the results obtained from field survey data. In relation to relevant factors influencing sampled population and practice of ANC options, bi-variable and multi-variable analysis was carried out to see the effect of each independent variable on dependent variable. Finally texts, tables, and charts were used to present and appraise the results. Results: The prevalence of antenatal care utilization among reproductive age women was 91.2% (95%CI: 88.2%, 93.5%). Maternal age of 25-35years (AOR=-6,46--; 95%CI: 1,66, 25.04), maternal occupation (AOR=2.10; 95%CI: 1.02,19.80 ), planned pregnacy (AOR=4.37; 95%CI: 1.35, 14.12), source of information about ANC (AOR=5.66; 95%CI: 1.56, 20.46), and partner educational status of colledge and above (AOR=4.35; 95%CI: 2.21, 8.53), knowledge about danger signs of pregnacy (AOR 2.91 ;95%CI:1.30,6.74), monthly income of the family (AOR=2.77;95%CI: 1.53, 8.14) significantly associated with antenatal care utilization. Conclusions and recommendations: A significant percentage (91.2%) of the respondents were ever attended antenatal care. Maternal age, maternal occupation, planned pregnacy, source of information about ANC, knowledge about danger signs of pregnacy and partner educational status were significantly associated with antenatal care utilization. Further improvement of antenatal care utilization focusing on the identified factors is recommended.
