Background: Despite availability of low-cost vaccines against tetanus, one newborn baby dies every nine minutes due to tetanus. And, thousands mothers also die due to this disease globally. Tetanus toxoid immunization is provided free of charge in Ethiopia but only 49% of mothers received two tetanus toxoid injections during their last pregnancy. As a result, 51% infants were not protected at birth against tetanus. Even though studies have been conducted on tetanus toxoid and associated factors findings are inconsistent in Ethiopia. Objective: To assess tetanus toxoid vaccination status and associated factors among mothers who gave birth within one year prior to the study in Machakel district, North West, Ethiopia. Methods: A community based cross sectional study was conducted from March 02-20/2020 in Machakel woreda, North West Ethiopia. Stratified multi stage sampling technique was employed. There were 28 kebeles in the woreda and stratified in to urban and rural kebeles. Then one urban and six rural kebeles were selected by simple random sampling method. Finally, simple random sampling technique was employed to select 634 participants. Data were collected by face to face interview using structured and pre-tested questionnaire. Data were coded and entered into EpiData 3.1and then was exported to SPSS version 20.0 for analysis. Variables with P-value less than or equal to 0.25 in simple binary logistic regression were fitted in to multiple binary logistic regression analysis. Finally adjusted odds ratio with 95%CI and p-value less than 0.05 was computed to determine the level of significance. Hosmer and Leme-show test with p-value greater than 0.05 was checked for model fitness. Result: About 64 %( 95% CI= 59.9, 67.7) of mothers reported that they had received tetanus toxoid vaccination. Attending formal education(AOR=2.28,95% CI= 1.10,4.72), attending four or more Antenatal care visits (AOR=3.11,95%CI= 1.45-6.70),attending pregnant women conference (AOR=3.18,95% CI= 1.73,8.38),less than one hour to reach the nearest health facility (AOR=2.86,95% CI= 1.23,6.64),visited by extension worker (AOR=2.94,95% CI= 1.36,6.37)being informed about TT vaccination (AOR=2.38,95% CI= 11.05,5.05) were significantly associated with two or more tetanus toxoid vaccination. Conclusion and recommendation: The study showed that the coverage of women who received two or more tetanus toxoid vaccinations was lower than the national target. Strengthening health education regarding tetanus toxoid vaccination, strengthen antenatal service use, programed health extension home visit and participation of pregnant women conference are recommended.
